The following items are necessary for completing the Charter process.  Each is needed not only to initially charter but also to renew a charter.  The chart below lists the item, the link to the item and description of the item.

Screening Requirements
Forms for Leaders including applications, references, interview and agreement form

Certified Overnight Chaperone Training Agreeement
Form found in the 4-H Overnight Chaperone Trainer's handbook and provided by the trainer following completion of the program

Club Leader Training
Completed at the local level using the outline provided here OR completion of the 4-H Certified Club Leader Training. Agreement provided by training coordinator

USDA Name & Emblem Requirements
From National 4-H Headquarters, guidelines and use for the 4-H name and emblem


4-H Code of Conduct Forms
One per member. Original to remain in the county office with copy to 4-H Club

4-H Enrollment Forms
May use ones from the County Extension office rather than the link provided.  Must have one on each member with new members forwarded promptly to the Extension office

4-H Club Action Plan
The form provided is a guide. Others may be substituted as long as the principle items listed are included

From National 4-H Headquarters, the 4-H Essential Elements provide the foundation for 4-H club programming
